Help w controls for left AK amp

Got clipped in 05 and ended up left AK amputee. After looking at the options I want to swap the clutch and rear brake controls and add a jockey shift. That is, right side foot clutch and left hand rear brake. 04 Ultra. I will need custom length hydraulic hoses and the shifter. Advice where to buy? Thank you.
 
Same thing happened to me after hitting a deer in 2014. AK left amp. i tried riding a 2 wheeler but just could not do it and ended up selling my other bike and buying a tri-glide. I turned the rear shifter up and welded and rod on it and installed a shift knob and been riding this way since 2015. A little slow to shift but works for me. I am always ready for new ideas and keep my eyes open for them. Anything new you come up with please post here.
